Claim Requirements

All claims shall be filed together with reasonable proofs of death and all supporting information.

Notice of claim must be submitted to OWDEE within thirty (30) days and proof of loss within ninety (90) days from the date of death, except when it can be shown that failure to submit the notice and proof within such time is due to valid reason.

The following are the basic requirements on claiming the insurance benefit. OWDEE has the right to require additional requirements on a case to case basis.

Life Insurance Accidental Death Accidental Dismemberment Hospital Income Total and Permanent Disability

Life Insurance

Basic requirements on claiming the Life Insurance.

  • Original Certified true copy of Death Certificate
  • Notarized Claimant’s Statement
  • Attending Physician's Statement
  • Marriage Certificate or Birth Certificate of the beneficiary
  • Affidavit of Two Disinterested Persons (if necessary)
  • CENOMAR (of both insured and claimant)
  • Barangay Certificate (indicating their years of living together), if claimant is a common-law-partner

Accidental Death

Basic requirements on claiming the Accidental Death Benefit.

  • Original Certified true copy of Death Certificate
  • Notarized Claimants Statement
  • Original Certified true copy of Death Certificate
  • Marriage Certificate or Birth Certificate of the beneficiary
  • Affidavit of Two Disinterested Persons (if necessary)
  • Original Police Report or Barangay Accident Report
  • CENOMAR (of both insured and claimant) and Barangay Certificate (indicating their years of living together), if claimant is a common-law-partner

Accidental Dismemberment

Basic requirements on claiming the Accidental Dismemberment Benefit.

  • Notarized Claimant’s Statement
  • Original Police Report or Barangay Accident Report
  • Original Medical Certificate indicating final diagnosis.


Hospital Income

Basic requirements on claiming the Hospital Income Benefit:

  • Original Medical Certificate indicating final diagnosis
  • Original Hospital Bill
  • Statement of Account
  • Police/Incident Report (if claim is not due to natural cause)
  • Affidavit of Two Disinterested Persons (if necessary)


Total and Permanent Disability

Basic requirements on claiming the Total and Permanent Disability Benefit:

  • Original Police/Incident Report
  • Original Medical Certificate indicating final diagnosis
  • Surgical Report (if a surgery was done)
  • TPD Questionnaire
